1. Primary
Dysmenorrhea
This is common menstrual cramping that isn't caused by another pelvic disease. It typically begins within a year or two of a girl getting her first period. The pain is driven by a natural chemical called prostaglandin, which makes the muscles of the uterus contract to shed its lining. High levels of prostaglandins cause sharper, more painful contractions.
2. Secondary
Dysmenorrhea
This refers to painful periods caused by an underlying medical condition involving the reproductive organs. Unlike primary dysmenorrhea, the pain often starts earlier in the menstrual cycle, lasts longer than normal cramps, and can worsen over time.
As illustrated in the medical diagram above, structural conditions like endometriosis cause tissue similar to the uterine lining to grow on areas like the fallopian tubes and ovaries. This tissue swells and bleeds during your period with no way to exit your body, leading to intense inflammation and severe menstrual cramps.
